Most surgeries are under anesthesia. However, a patient can choose to have epidural anesthesia.
The operation is performed in one stage procedure, consisting of vaginoplasty, outer labioplasty, inner labioplasty, urethroplasty and sensate neoclitoplasty Orchiectomy; our surgeon performs orchiectomy in every operation of SRS in order to prevent production of male hormones.
Depth of neo – vagina will be 5 – 6 inches on average for a western male. However, in some cases, immediate result after surgery could be up to 7 – 8 inches. Long term results are different depending on many factors such as dilation and wound care.
Erectile tissue removal; surgeon makes special attention to remove the erectile around urethral opening as much as possible, to prevent the excess skin and erectile tissue below the urethra.

Disadvantage: It is not suitable for men with penises shorter than four (4) inches, because this will result in a vagina that does not have satisfactory depth. Skin Graft Technique (SRS2)

If after the scrotal skin graft, the vagina depth is still not satisfactory to the patient and then the plastic surgeon will consider using skin graft from other areas, such as the upper limbs, groin, to further increase the depth of the vagina.
Advantage: This technique helps patients with short penis and enables them to process the desired vaginal depth.
Disadvantage: The surgery is difficult and complex, the surgery time will be longer, for this technique experienced and specialized plastic surgeons in sex reassignment surgery will take about six (6) hours to complete.

Sigmoid Colon Vaginoplasty Technique (SRS3)

This technique is used in the case when patients have short penis or patients whose vaginas have obstructed. This can also be used in patients who have never undergone sex rearrangement surgery; the vagina which is part of the colon will have a good lubricant.
The average vaginal depth depends on the penile and sigmoid part, the average length of sigmoid colon would be approximately 7-8 inches and there will be an 8-10cm scar on your abdomen. The operation will take about 7 to 8 hours.
Advantage: This technique helps patient with short penis, in this regards, the surgeon will decide the surgery type, SRS1 and SRS2 cannot be perform, the vagina has natural lubricant, it is possible to determine the depth of the newly constructed vagina.
Disadvantage: A scar of approximately 8-10cm long may be visible above the left side of the pubis, the surgery is difficult to complete with preparation procedures required such as, cutting of parts of the colon and the colon must be thoroughly cleansed (by enema) one (1) day prior to the surgery.The patient may experience dyspepsia/indigestion symptoms 2 – 3 days after surgery.

Sigmoid Colon Vaginoplasty – for correction

To have more cavity depth, you need Sigmoid Colon technique. This technique can achieve 6 – 7 inches. However this varies each individual. A vaginal from colon segment has no more bowel content passing through and less bacterial population.
